Findings: The Alleged Victim (AV) and Witness #1 (W1) are care planned to be separated when seen together. On or about February 14, 2022, Alleged Perpetrator #2 (AP2) was to keep eyes on AV and W1 and ensure they were separated if seen together. AV and W1 were standing together talking when W1 slapped AV. Neither resident were hurt, however, both were placed at risk for harm. AP2 failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facilty's failure is a violation of Oregon Administrative Rules.
