- Findings
- The facility failed to implement interventions and appropriately care plan regarding Alleged Victim’s (AV) known fall risk. AV fell on or about February 22, 2022, resulting in a head injury and AV being transferred to the hospital for treatment. The facility failed to implement person centered interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
