Findings: The facility failed to assess and intervene according to the Alleged Victim’s change of condition and when s/he began to have an increase in falls. AV’s physician recommended scheduling a cardiology appointment and later for AV to be evaluated, yet there is no documentation from the facility that either recommendation was followed. Between approximately February 4 and March 14, 2022, AV experienced three falls with injury, without facility interventions. The facility’s failure to provide appropriate services to AV resulted in him/her experiencing repeated injury, and not receiving necessary evaluation for his/her medical condition, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
