Findings: The Alleged Victim (AV) returned to the facility from the hospital on February 3, 2022, with a wound that needed care completed by staff. AV has extensive needs regarding activities of daily living, including toileting assistance and personal hygiene. AV's room was noted to smell strongly of urine and chart notes state staff put wet blankets back on AV's bed. AV had an incontinence incident during night shift, the next day, AV had a medical appointment, to which AV's doctor noted that AV had fecal matter on his/her wheelchair and up his/her back and legs. There is a two week gap in AV's chart notes, therefore, any care or lack of care was not documented. The faciilty failed to provide service to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
