Findings: According to documentation the facility failed to follow the Alleged Victim (AV) care plan to have oral care assistants 2x daily. The failure resulted in AV not receiving oral care, on or about February 15, 2022 - February 23, 2022, experiencing unreasonable discomfort and a loss of dignity. The facility failed to ensure care plans were followed, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
