Findings: On or about February 28 ,2022, the Alleged Victim (AV) wandered out of locked Memory Care unit and was found by a staff member wandering in the middle of the road. AV had been known to exit seek, however, got out of the unit. AV being outside of the locked unit places AV at risk for harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
