Findings: The Alleged Victim (AV) is a fall risk and has a history of falls. According to an investigation, between February 2, 2022, and February 26, 2022, AV experienced approximately four falls in h/h apartment. The AV sustained bruises on h/h finger, wrist, and elbow, skin tears on h/h arms, and a rib fracture, resulting in repeated unreasonable discomfort. The facility failed to provide appropriate services, relating to care planning and lack of appropriate interventions. This failure is a violation of resident’s rights, is neglect of care and constitutes abuse.
