Findings: The Alleged Victim (AV) is a fall risk and has a history of falls. According to an investigation, on or about February 27, 2022, the AV fell six times in h/h apartment between January 18, 2022, and February 26, 2022. The AV sustained abrasions, skin tears, and an ankle injury, resulting in repeated instances of unreasonable discomfort and the inability to bear weight. The facility failed to appropriately care plan and implement fall interventions. This failure is a violation of resident’s rights, is neglect of care, and constitutes abuse.
