Findings: Alleged Victim (AV) is care planned to receive two meals a day, delivered to his/her room. On several occasions, the facility failed to deliver AV his/her meals, resulting in AV going without food or receiving only a sandwich, which resulted in unreasonable discomfort. The facility failed to provide proper food/nutrition to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
