Findings: According to documentation and the investigation, on or about February 24, 2022, Alleged Perpetrator 2 (AP2) touched the Alleged Victim's (AV) breasts. AV experienced unreasonable discomfort and a loss of personal dignity. AP2's actions are considered sexual contact and constitutes abuse. The facility failed to protect a resident from inappropriate sexual contact, which is a violation of Oregon Administrative rules.
