Findings: On or about March 2, 2022, the Alleged Victim (AV) was feeling ill and had a bout with diarrhea, which ended up all over AV's bed, self and blankets. Av's room smelled of feces. AV called for help but no one came to help AV. Alleged Perpetrator #2 (AP2) was on shit that night and responsible for the care of AV. AV's care plan states that AV was to have safety checks every two hours. AP2 could not verify that he/she actually checked on AV that often. It is unknown how long AV was laying in his/her own feces that evening, however, this action placed AV at risk for harm and in unreasonable discomfort. AP2's actions are a violation of resident rights, are considered neglect of care and constitutes abuse. The facility failure is a violation of Oregon Administrative Rules.
