Findings: On or about March 4, 2022, the Alleged Victim (AV) left the facility when visitors held open the door for AV to leave. AV left the facility and was found by a police officer, who took AV to his/her spouses home. AV was returned to the facility without injury, however, AV was placed at risk of harm by leaving the facility unnoticed by staff. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
