Findings: Alleged Victim (AV) is a known fall risk. Between January 28, 2022, and March 11, 2022, AV experienced approximately three (3) falls in the same fashion, resulting in a hand laceration, and two head injuries, causing AV unreasonable discomfort. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
