Findings: Alleged Victim (AV) is a known fall risk and is care planned to have bed and wheelchair alarms. On or about March 5, 2022, AV had a fall resulting in a broken wrist, and it was noted that AV’s bed alarms were not in use at that time and that the facility was understaffed. The facility failed to follow AV’s care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
