Findings: Alleged Victim (VA) has a history of altercations with Witness 1 (W1) which have taken place on or about May 14th, July 6th, and November 19, 2021. On or about March 3, 2022, staff heard a noise in the dining room, staff found AV pulling on W1’s wrist and arm. W1 had started to pick up cups and AV did not like that W1 was picking up the cups and so begun to grab and pull W1’s wrist and arm. The facility failed to provide a safe environment for W1 and putting W1 in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse. The facility failed to care plan arounds AV’s known history of altercations, which is a violation of resident rights is neglect of care and constitutes abuse.
