Findings: On or about March 6, 2022, the Alleged Victim (AV) was attending Mass and exited the facility unattended. AV was unattended at Mass by any staff, resulting in him/her exiting the building unattended, placing AV at risk for harm. AV was found and brought back to the facility unharmed, however, AV was at risk. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
