Findings: The Alleged Victim (AV) relies on the facility for his or her safety. On or about March 12, 2022, AV was found outside the locked facility by a staff member who was on his/her way to work. The working facility staff were not aware AV had eloped from the facility, crossed a busy five (5) lane road and was approximately one and one-half blocks away, placing AV at risk for serious harm. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00601 $338.00 fine assessed
