Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about March 15, 2022, AV’s wallet went missing. The wallet had $1,000.00 in cash in it. Staff helped AV search for the wallet and the cash, but they could not find them. It is believed an unknown Alleged Perpetrator 2 (AP2) may have taken AV’s wallet and cash. AP2’s action is considered financial exploitation, which is neglect of care and constitutes abuse. The facility failed to protect AV from financial exploitation, which is a violation of Oregon Administrative Rules.
