- Findings
- AV has a history of falls and frequently falling forward out of h/h wheelchair. On or about March 11, 2022, AV fell forward from h/h wheelchair, and sustained a skin tear to upper lip. The facility did not have a service plan in place, which provided interventions, care and services appropriate for AVs continued falls out of h/h wheelchair, which is a violation of resident’s rights is neglect of care and constitutes abuse.
