Findings: The Alleged Victim (AV) and Witness 1 (W1) have history of behaviors and resident-to-resident altercations. AV is care planned for staff to escort AV in the elevator alone with no other residents to be present due to behaviors. On or about March 20, 2022, AV was in the elevator alone and W1 was standing outside of the elevator when a resident-to-resident incident occurred and AV fell back into the elevator railing, causing AV to receive a skin tear to his/her right forearm. The facility failed to follow the care plan, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
