Findings: The facility, Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) failed to provide a safe medication administration system to ensure the Alleged Victim's (AV) medication was administered as ordered. AV was unresponsive, transported to the hospital and stayed overnight. The facility’s, AP2’s and AP3’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
