Findings: On or about March 18, 2022, the Alleged Victim (AV) was being transferred by facility staff from a recliner to his/her wheelchair. AV is care planned to be a two person transfer, however, staff transferred AV alone. During the transfer, AV received a skin tear on his/her leg. AV was transferred to the hospital where he/she received wound care and returned to the facility. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
