Findings: Alleged Victim (AV) relies on the facility for his/her care. The facility failed to provide oversight and monitoring of AV change of condition. The failure resulted in AV not receiving interventions being put into place which resulted in dehydration, pressure wounds and AV being sent to the hospital. The facility failure is a violation or resident rights, is considered neglect of care and constitutes abuse.
