Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about April 27, 2022, Witness 1 (W1) hit/slapped AV in the face again. AV and W1 have had several physical resident-to-resident altercations. A temporary service plan from incident date of March 22, 2022, outlined for staff to keep AV and W1 separated. During the incident of April 27, 2022, AV and W1 were sitting with each other at the same table after dinner. The facility failed to follow the service plan, which is a violation of resident rights is neglect of care and constitutes abuse.
Sanction: RCFCP22-01452 $750.00 fine assessed
