Findings: According to the documentation, the facility failed to provide a safe environment to ensure the Alleged Victim (AV) could not elope from the facility. On or about March 23, 2022, the AV was seen walking down the street away from the facility. The AV was having a hard time breathing and needed breathing medication. Review of the video from the courtyard showed the AV used a table to jump over the fence, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
