Findings: On or about March 9, 2022, the Alleged Victim (AV) was found on the floor in his/her kitchen. AV complained of a bruised jaw and pain in his/her toe after the fall. Staff who assisted AV up did not complete any assessment of AV after the fall to ensure no injuries. The facility's failure to assess after a fall is a violation of resident rights, is considered neglect of care and constitutes abuse.
