Findings: Alleged Victim (AV) service plan dated 12/07/2021 indicates Minimal assist and verbal prompts/cues for toileting tasks. AV had a change in toileting needs and refusal behaviors. The facility failed to appropriately reassess and care plan according to AVs needs, which is a violation of resident’s rights is neglect of care and constitutes abuse.
