- Findings
- On or about March 24, 2022, it was discovered AV did not receive h/h medications as ordered. The facility failed to provide a safe medication administration system to ensure the Alleged Victims (AV) medication orders were administered as prescribed, which is a violation of Oregon Administrative Rules.
