Findings: Alleged Victim (AV) relies on the facility for a safe environment. Witness 1 (W1) has history of behavioral changes including aggressive behavior towards staff and other residents. The facility failed to adjust the service plan for W1 leading to continued aggressive behaviors. On or about March 27, 2022, W1 entered another resident’s room and hit that resident in the face. Then W1 entered the hallway hitting AV and another resident in the arm. The Facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-01884 $375.00 fine assessed
