- On or about April 2, 2022, Witness #2 (W2) took the Alleged Victim (AV) to the ground because AV was being vocal and W2 was irritated. W2 has a history of hitting residents. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
