Findings: The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #1 (W1). An incident occurred between W1 and the Alleged Victim (AV) where W1 attempted to enter AV's room and ran over AV's feet with his/her walker, causing bruising to AV's ankle. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
