Findings: The facility failed to implement interventions and provide appropriate supervision related to the Alleged Victim’s (AV) known history of falls. The failure resulted in AV experiencing an unwitnessed fall on 4/1/2022, was transferred to the hospital and was diagnosed with several fractures in his/her back and incisors. Prior to the serious injuries AV sustained on the 4/1/2022 fall, AV suffered 7 falls within 60 days. Out of the 7 falls AV experienced, AV sustained either injury, pain, or emergency room visit on 4 of the 7 falls. The facility's failure to properly care plan for AV's known fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
