Findings: Alleged Victim (AV) is care planned as independent mobility with the use of h/h walker or wheelchair. AV has history of non-compliance with community rules. AV has history of balance issues, has a diagnosis, and takes medications that affect balance and contribute to falls. On or about April 02, 2022, AV had an unwitnessed fall out of h/h wheelchair that resulted in a fracture injury. The facility failed to properly plan care around AV’s needs and provided interventions appropriate to mitigate AV falls which is a violation of resident rights, is neglect of care and constitutes abuse.
