Findings: According to documentation the facility failed to follow The Alleged Victim’s (AV) care plan to apply numbing gel on AV’s gums, to reduce av’s pain while chewing. The failure resulted in AV losing thirty pounds over the course of from on or about November 2021- April 08, 2022, from not being able to eat, and AV experiencing pain and unreasonable discomfort. The facility failed to ensure care plans were followed, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
