Findings: According to the documentation, the facility failed to properly care plan appropriate fall interventions for the Alleged Victim’s (AV) known fall history. From approximately March 27, 2022, through March 29, 2022, the AV suffered approximately three falls, two which caused injury. On or about March 29, 2022, after the fall, EMS was called due to the AV experiencing shoulder pain. The failure to properly care plan appropriate fall interventions for the AV’s known fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
