Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to his/her wound care, assistance with mobility to and from meals, assistance with bathing and personal hygiene, and assistance with eating/nutrition. AV experienced worsening of skin injuries, oral thrush, missed showers, fractured toe, unsupervised meals and did not receive a mechanical soft diet. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
