Findings: Alleged Victim has history of falls due to not having h/h mobility device within reach. On or about April 10, 2022, AVs mobility device was not within reach resulting in AV falling, hitting h/h head and going to the emergency room. The allegation that AP2 failed to follow care plan was investigated and the determination was not substantiated. The facility failed to properly plan care and provided interventions appropriate to mitigate AV ongoing falls which is a violation of resident rights, is considered neglect of care and constitutes abuse.
