Findings: The Alleged Victim (AV) relies on the facility for his/her care and is a known fall risk. The facility failed to keep resident safe, care plan appropriately, and provide services according to the Alleged Victim (AV)'s needs, relating to falls and lack of appropriate interventions. According to an investigation, AV experienced approximately twenty-two falls between December 9, 2021, and February 17, 2022. AV experienced repeated unreasonable discomfort from varying degrees of injuries, including traumatic subarachnoid hemorrhage, left orbit fracture, and sinus fracture. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
