Findings: Based on facility documentation and interviews, it was determined that Alleged Perpetrator 2 (AP2) forged Alleged Victim's (AV) Medication Administration Record, giving the appearance that AV's pain medication was administered as ordered and without error. AP2 was assigned to administer AV's medication daily on April 8, 2022, April 9, 2022, April 10, 2022, and April 11, 2022 and failed to follow through with his/her medication administration assignment for AV's pain medication. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administration Rules.
