- Findings
- Alleged Victim (AV) has a known history of falls. Between March 29th, through April 14th, 2022, AV suffered 12 falls, several falls with injury, including a nasal bone fracture. On or about April 17, 2022, AV fell twice, once in the dining room and once in the activity room. The facility failed to appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
