Findings: On or about April 14, 2022, Alleged Victim (AV) discovered an unauthorized charge on his/her credit card in the amount of $528.00. Based on interviews and facility documentation, it was determined that AP2 financially exploited AV by wrongfully taking funds belonging to AV which resulted in financial loss to AV. AP2's actions is considered financial exploitation which constitutes abuse. The facility failed to protect AV from theft which is a violation of Oregon Administrative Rules.
