Findings: The Alleged Victim (AV) relies on the facility for his or her safety. AV’s service plan is noted for elopement risk and exit seeking behaviors. On or about April 18, 2022, AV eloped from the facility, and walked into traffic. AV was brought back to the facility by Law enforcement. The facility failed to provide a safe environment and prevent elopement, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
