Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about April 18, 2022, AV pushed call light for assistance to the bathroom, and waited approximately an hour, causing unreasonable discomfort. The facility failed to answer call light in a timely manner, which is a violation of resident rights, is neglect of care and constitutes abuse.
