Findings: Alleged Victim (AV) relies on the facility for h/h care. On or about April 15, 2022, it was discovered that AV needs were not being met by the facility to include bathing, grooming, dressing, responding to resident call lights, and resident monitoring. The failures placed AV at risk of serious harm. The facility failed to provide a service, which is a violation of resident’s rights, is neglect of care, and is constitutes abuse.
