- Findings
- The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to his/her change of condition, rapid weight loss and behaviors. AV experienced significant weight loss, skin injuries as a result of behaviors and went without medications. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
