Findings: The facility failed to implement interventions and appropriately care plan regarding Alleged Victim’s (AV) known fall risk. On or about April 21,2022, AV was found on the floor and was sent to the hospital for elbow pain and a head laceration. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which caused repeated unreasonable discomfort, is a violation of resident rights, is considered neglect of care and constitutes abuse.
