Findings: The facility failed to protect the Alleged Victim from financial exploitation. On or about April 21, 2022, it was discovered that the Alleged Victim's (AV) debit card was used in two different locations, two different days. Alleged Perpetrator #2 (AP2, Unknown) is responsible for taking AV's debit card. AP2's actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility's failure is a violation of Oregon Administrative Rules.
