Findings: According to documentation the facility failed to follow the Alleged Victim’s (AV) care plan. AV’s care plan states AV needs to wear h/h wander guard on h/h wrist due to wandering and elopement risk. The failure resulted in AV being found by security outside of the facility, on or about April 24, 2022, putting AV in risk of harm. The facility failed to ensure care plans were followed, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
