Findings: On or about April 22, 2022, Alleged Victim (AV) was found deceased on the floor of his/her bathroom floor. Based on facility documentation and interviews, there is sufficient information available to indicate that the facility failed to provide adequate care of AV by not completing thorough safety checks within the hours of 1400, when AV was last seen and 2100 hours, when EMS was called after facility staff had checked on AV. The facility failed to follow AV's care plan requiring 2-3 hour frequent safety checks resulting in serious risk of harm to AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
