Findings: The Alleged Victim (AV) had approximately eleven (11) falls from February 05, 2022, to April 25, 2022, from which AV was diagnosed with fractured right and left hip and left femur. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
